Privacy Policy
Thank you for visiting the PAF web site and for reviewing PAF's Privacy Policy. PAF's policy on the collection of personal information is as follows:
Collection of Information
Unless you provide additional information, PAF collects only the following information as you browse through the web sites under the PAF umbrella:
- the domain name and browser you use to access the Internet;
- the date and time of your visit;
- the pages you visited; and
- the address of the web site you visited immediately prior to visiting the PAF site.
If you send an e-mail message to PAF or complete a questionnaire, form, or other online survey found on the PAF web sites, PAF will maintain the information in accordance with applicable federal law. PAF collects information that you volunteer through electronic forms such as "Contact Us" and "Personalized Help". Certain information is required to process your request. When information is requested from you, PAF attempts to clearly indicate which information is required and which information is optional. Information collected to process specific transactions is only used for the specific service that you have chosen. However, throughout PAF Web sites you are offered opportunities to receive related or general information in the future from PAF. In these cases, PAF will ask for your "permission" to contact you.
PAF encrypts questionnaires, forms, or other online surveys found on the PAF web sites that ask for specific patient information and acts in accordance with HIPPA compliance. PAF will store your personal information in internal secured databases. PAF will update our databases periodically to ensure that it remains current, and will purge your Personal Information when PAF believe it is no longer valid or current.
PAF does not disclose, give, sell, or transfer any personal information about PAF web sites' visitors unless required for law enforcement or otherwise required by law.
Security
PAF has taken the appropriate precautions to protect information sent over the Internet. PAF web sites employ strict security measures to safeguard and protect against the loss, misuse, and alteration of the information. For site security purposes and to ensure that this service remains available to all users, PAF employs software programs to identify unauthorized attempts to upload or change information, or otherwise cause damage. Any personal information is stored in a secure database and sent and received by a secure, encrypted Internet channel wherever possible. PAF staff must follow this privacy policy; those who violate it are subject to disciplinary action, up to and including termination.
Your Email
If you contact us by email, your message will be forwarded to the PAF staff member best able to address your questions and concerns. Please note that email transmissions over the Internet with PAF are not encrypted.
Non-encrypted Internet email communications are not completely secure or private and may be intercepted and viewed by other Internet users, without your knowledge and permission, while in transit to us.
To protect your privacy, please do not use email to communicate confidential information. Please choose to contact PAF via web forms or by telephone for matters that require greater privacy and security.
This site is maintained by the Patient Advocate Foundation and is protected by various provisions of Title 18, U.S. Code. Violations of Title 18 are subject to criminal prosecution in Federal court.
Use of Cookies - Certain PAF webpages use cookies which are small amounts of information stored by your web browser software on your computer. In most cases, this information is temporary in nature (called session cookies) and is deleted as soon as you leave the PAF web site. The use of temporary cookies is solely for the purpose of allowing the user to interact with the PAF web site in an effective manner. In a few cases, the PAF web site uses cookies to store information for a longer period than the current session (persistent cookie). In those cases, the webpage is clearly noted and the user can choose not to use that webpage feature.
